- Internal review leakage — reader-facing Docs/Preview/fixture copy exposes Hallmark/Semantic-wrap/anti-slop pass results, migration/promotion gates, source/static review status, authoring-tool narration or other internal QA scaffolding. Keep these in repository review docs, Journey or test output instead.

- Card soup — containment is the default separator for every idea
- Badge salad — multiple pills compete without state hierarchy
- Selection boxing — current/approved content gets a full outline or card merely to say “selected”; prefer local label, restrained background, proof mark or editorial rule when no ownership boundary exists
- Dashboard theatre — equal KPI cards whose values do not change the next decision
- Eyebrow tic — small uppercase metadata before every heading
- Semantic color flood — large tinted surfaces for every state
- Architecture leakage — canonical/materialized/runtime internals in primary product UI without user value
- Landing-page gravity — oversized hero typography/marketing rhythm inside a workbench
- Template fingerprint — same radius/padding/card grammar everywhere
- Semantic flattening — why/risk/unknown or fact/action/provenance forced into identical containers
- Ambient garnish — aurora/radial gradients, glow, floating decorative blobs, generic AI sparkle
- Universal hover lift — translate/scale motion on every clickable tile
- Fake chrome — redrawn browser/IDE/device frames with no product meaning
